(1) Applicant’s name, address, phone number, and the university or institute of attendance as of January 1, 2018.
(2) ESSAY QUESTION:
Did you ever participate in a Youth Sports Program? If so, which one(s)? How has participating in youth sports programs helped your character development? Did you feel any joy of accomplishment? What life skills did you learn?
