Requirements:
- Secure a copy of your school enrollment slip. This will be your proof that you are indeed a student at the time you send your application.
- Write a short biography about yourself. Allot a separate page for basic personal information that should include: Full Name, School, Age, Home Address, and Contact Number.
- Write a compelling essay of a minimum of 1000 words. The essay topic is “Better Daily Life for People with Scoliosis” to raise awareness about the illness. It should critically discuss the difficulties of living with Scoliosis, what practices or activities people with scoliosis can do to better their lives, and daily choices that could lead to improvement in quality of life. You may also discuss what families, loved ones, and friends could do better to help ease the daily struggles of living with scoliosis.
