Award Amount: $1,500


Requirements: 

• Complete the Future Medical Professional Scholarship form found on our website. (Click here to download the form).
• Submit a 650-word essay about your future goals in the medical field.
• A transcript reflecting your current academic standing, a cumulative GPA of at least 3.0, and proof of enrollment.

Submit all of these materials by mail or via e-mail. Failing to include all of these materials will result in your application being rejected.
To apply for the scholarship online, submit your application and materials attached to an email to Prosper Shaked at scholarships@prosperlaw.com. Your email’s subject line should be “Prosper Shaked Scholarship for Future Medical Professionals.”
